In working with my pediatric community client it was my responsibility to document occupational therapy services to ensure accountability. It was required that these notes meet standards for reimbursement and adhere to the requirements of the Life Skills Clinic as well as local, state, federal, and reimbursement agencies. It was impressive that the documentation represented the context in which the services were delivered, effectively communicate the need rational for continuing occupational therapy services.
